Chronic suppurative apical periodontitis: A clinical case report

ABSTRACT

The purpose of this clinical case is to present a surgical procedure in which important teeth are preserved in the mouth to act as support for a fixed prosthesis. The patient in question had pulpal necrosis with longstanding chronic non-suppurative apical periodontitis. Initially, root canal work was carried out on the lower central incisors. The patient did not attend any control visit for 2 years, until she returned with a more severe apical lesion and a definitive diagnosis of chronic suppurative apical periodontitis. In this case, the patient did not want her fixed prosthesis to be changed or perforated and, given the marked degree of bone loss at teeth 24 and 25, a decision was made to treat the condition surgically. An apicoectomy was performed using cold-burnished gutta-percha and a particulate bone graft. The patient was then monitored over a period of three years, during which time clinical observation revealed very healthy tissue with no fistulas and X-ray analysis showed bone regeneration indicative of a positive prognosis.
