Proximal humeral fracture-dislocation in four parts with dehiscence of humeral head

ABSTRACT
Introduction: Of all fractures, the frequency of proximal humeral fractures is 5%; in people under 40 years is 45%, rising to 76% in the fifth decade; its incidence in adults is 105 per 100,000/year, more frequent in females (2:1); their treatment requires open reduction on many occasions, the circulation to the humeral head is provided by a single artery, the risk of avascular necrosis is 34%.
Case report: Male 62 years with four-part fracture of the humeral head, dehiscence and dislocation, managed with open reduction and osteosynthesis, stabilization sling and abduction pillow at 45°, managed in early rehabilitation; sling removal at 40 days to exercise; final with drawal after 60 days, sent home at 120 days with a score of 12 on the DASH scale, with restriction in external rotation.
Conclusions: The management of these lesions is with open reduction and pinning far from the glenoid; immediate postoperative rehabilitation with electrical stimulation prevents muscular disuse atrophy and stasis edema and allows for early mobilization.
