Pregnancy after ICSI with immobile sperm, posthypo- oosmotic test

ABSTRACT

This paper reports a case of intracytoplasmic sperm injection (ICSI) with immobile sperm obtained by testicular sperm extraction (TESE) in a patient with congenital bilateral absence of the vasa deferentia (CBAVD), heterozygous for the mutation 2789+5G›A of the CFTR gene of cystic fibrosis, which is the most common genetic cause of obstructive azoospermia and is present in up to 98% men with cystic fibrosis. During the cycle of ICSI, the hypo-oosmotic swelling test (HOS) was used for the identification of viable spermatozoa in the sample with total astenozoospermia. Five oocytes were injected, with fertilization in 4 and cleavage in 3, which were transferred at 48 h. The pregnancy test was performed on days 14 post-transfer reporting 244.1 UI/mL of β-HCG, a viable embryo was confirmed by ultrasonography. Although only one successful case was reported with ICSI using non-motile sperm combined with HOS, this technique is recommended as an alternative to the use of pentoxifilline to identify viable sperm with a realistic chance of achieving fertilization, embryonic development and normal pregnancy in patients with total astenozoospermia.
