ABSTRACT

We present the case of a previously healthy 33 year old male patient that showed to the emergency department with a 6 month history of effort angina. Primary evaluation revealed negative cardiac biomarkers, a Bruce protocol stress test was positive for ischemia. Coronariography revealed a muscular bridge that affected the anterior descending artery, treatment with calcium channel blockers and beta blockers was begun. Muscular bridging is a congenital disease where a portion of an epicardial coronary artery lies beneath the cardiac muscle causing occlusion of the artery in systole, manifesting clinically as angina, treatment is based on calcium channel blockers, beta blockers and in some cases miothomy
