ABSTRACT

lntroduction. The hyperinsulinemic state has diverse effects in several metabolic pathways that favor the progression of the diseased involved in the metabolic syndrome (MS) and cardiovascular risk. The metabolic syndrome can retard the resolution of clinically significant macular edema (CSME), but the proportion of patients with CSME that llave MS is unknown. Material and methods. An observational, analytic, cross sectional, prospective and open study was conducted. Diabetics with CSME and any degree of diabetic retinopathy, of any gender, aged 40 to 80 years, with or without a diagnosis of arterial hypertension were evaluated. The proportion and 95% confidence intervals (C.l.) of patients wi MS was determined, and each MS criteria was compared in patients with and without MS using Wilcoxon's t test. Results. Fifty patients, mean age 60.4 years, 68% females; twenty one patients had proliferative diabetic retinopad1y in at least one eye. Forty five patients met d1e criteria of MS (90%, 95% C.l. 81.68 98.32). Conclusions. Up to 98% of the diabetics with CSME could present MS. The clínical parameters of MS could be evaluated during the specialty visit, and would determine the need of more specific evaluations from other specialists; this could initiate and interdisciplinary approach and decrease the progression of complications.
