Prevention of oral disease in patients with systemic disorders. Part I: Cardiovascular Disease

ABSTRACT
In medicine and dentistry, customary practice has evolved from treating local and systemic diseases as separate concepts to accepting the possibility of bidirectionality, where mutual effects, be they beneficial or harmful, are apparent between the two. Bidirectionality acknowledges that the effect of local and general processes may be two-way (i.e., synergetic); that the pathogenic development of each can be independent or similar and come together or appear at specific moments (parallel development). The third option is that one is a manifestation of the other (causation). In the future, when the interface between medicine and dentistry and vice versa is better understood and more widely accepted, and the related prejudices and preconceptions disappear, this third type of association may prove to be that accepted as the standard concept, given that the causal relationship can accommodate not only the various branches of medicine, but also psychological, environmental, and social aspects, all involving a deterministic action as regards health and, more importantly, well-being. The causal relationship improves on the prevailing notion of bidirectionality in the sense that the latter continues to be grounded in the conceptualization of the distinct subdivisions of dentistry and medicine. In years to come, this causal relationship will make it evident that holistic medicine is merely an interacting part of a complex scenario in which elements other than just the biological ones, such as physical, mental, emotional, and economic factors, play a role, so broadening the concept of health that prevails today. Furthermore, a selection of information and reflections is presented on the interface between periodontology (the study of inflammatory periodontal conditions) and a number of cardiovascular processes and diseases, such as atherosclerosis, arterial hypertension, and ischemic disorders (infarction, angina). This presentation is accompanied by tables containing information relating to the preservation of the state of health of patients prone to developing cardiovascular disorders and conditions (salutogenesis).
