Tratamiento combinado para úlcera venosa crónica: compresión, escleroterapia de perforantes guiada por ultrasonido e injerto de piel

ABSTRACT

Introduction: It is considered that 20% of the world population suffers of phlebopatyas; the etiology of the sores of the low members is venous in 90% of the cases. The treatment divides in medically and surgically, the doctor is effective with closing sore in 88% to one year, nevertheless with high rates of recurrence of 28% to two years. The surgical treatment described by Linton where closing is realized of perforates, with high rates of complications that go from 12-53% and recurrence from 6- 22%.The endoscopic treatment with cicatrisation of 88% to three months and recurrence of 28% to 21 months. The sclerotherapy of perforates guided as ultrasound has a rate of closing of 75.4% and a recurrence of sore of 32% to 20 months. Reviews have been realized in Cochrane on the use of leather grafts in sores where one has concluded that the leather graft increases the cicatrisation nevertheless with discharge recidivation.
Objective: To determine index of cicatrisation and recidivation of sore in patients submitted to treatment combined for sores phlebostatics.
Material and methods: Patients are included by venous chronic sores, deposited to Servicio de Angiologia y Cirugia Vascular of the HRLALM in the period from November, 2006 to May, 2007, clinical, without information of arterial disease, they surrendered to an ultrasound Doppler duplex identifying the presence of perforates insufficient, surrendered to sclerotherapy with polidocanol, guided for Doppler; it is realize application of graft of epidermis, taken of previous face of thigh, with local anesthesia, later it gives compression and follow-up.
Results: 63% treated each other a whole of 20 extremities rights and 37% left sides; we realize sclerotherapy of perforates, it is place graft, with an integration adapted in 95%, later it is apply press therapy (15-20 mmHg) giving him a follow-up to 21 weeks without recidivation.
Conclusions: With those combine method obtain a cicatrisation in short time, is ambulatory and cost down, suggest in patients. Our results are similar to the brought reported ones in the literature, as for closing sore, obliteration of perforates, integration of graft and complications. By what it is needed of major time of follow-up to value it recurrence.
