Method of selection of antihypertensive treatment

ABSTRACT

Background: the strategies for the treatment of hypertension have not achieved to control neither the figures of blood pressure nor the prognostic of the complications or over life. Objective: to propose an individual strategy that allows to a more complete characterization of the hypertensive patient. Development: the organical damage is a silent determinant process in its high lethality being useful to detect it in a subclinical stage. In the evaluation strategy it is considered the classification by categories, the cardiovascular risk factors, the damages of the target organs, the coexistent medical conditions and the haemodynamic disturbances which are features of hypertensive groups at the time of selecting and indicating the hypotensor and causes of secondary blood hypertension. Conclusions: the revision and analysis propose the variables to be considered to accomplish a more complete and individual characterization of the hypertensive patient and adequate selection of treatment being worthy to incorporate other risk factors as time of evolution, maintained emotional pressure and microalbuminuria. In target organ damages it has to be present the subclinical stage, multiple damage, metabolic syndrome and the vascular arterial damage. Besides to consider the coexistent medical conditions, haemodynamic characteristics in hypertensive groups in correspondence with the action mechanisms of the hypotensive medicaments for its selection, to incorporate new profitable diagnostic means for the detection in subclinical stage, stratification of the cardiovascular risk and causes of the secondary blood pressure to privilege the ones that are potentially curable.
