ABSTRACT
Background: With the advent of laparoscopy, endoscopy retrograde cholangiopancreatography and magnetic resonance imaging, the management of patients with choledocholithiasis has dramatically changed. There is no consensus on what is the algorithm for diagnosis and treatment of gallstones and the management of this disease depends on the current experience and potential of technological availability. Methods: We describe a case of a female patient aged 73 with multiple comorbidities and a diagnosis of choledocholithiasis not resolved by endoscopy. Results: We performed cholecystectomy and bile duct exploration, with removal of two lithos by choledochotomy and subsequent placement of a transcystic probe, intraoperative cholangiography choledochorrhaphy and without filling defects. Conclusions: The laparoscopic treatment of choledocholithiasis has demonstrated efficacy and safety in centers with experience in laparoscopic surgery, being a good option for the management of patients with residual choledocholithiasis.REFERENCES
