Acne fulminans in an adolescent with iatrogenic Cushing syndrome

ABSTRACT
Acne fulminans is a variant extremely rare and severe inflammatory acne, sudden onset and aggressive course associated with systemic symptoms, abnormal lab tests and often osteolytic lesions. Usually affects adolescent males with a history of mild to moderate acne. Because the etiology remains unclear, it has been suggested a multifactorial origin. So far very few cases reported in the literature, but it is recommended that treatment be aggressive from the start by combining glucocorticoids and oral isotretinoin. We report the case of a 17 years old previously healthy, with acne fulminans paradoxically triggered by prolonged administration of glucocorticoids and their abrupt suspension, with favorable response after administration of retinoids and antibiotics. Is important to emphasize that the clinical suspicion of this variant and its management as a systemic disease, as it does not respond to conventional therapy of acne vulgaris.
