Submandibular epidermoid cyst: a case report

ABSTRACT

Background: we present the case of a patient with a submandibular tumor, probably congenital, diagnosed on histopathology as an epidermoid cyst.
Clinical case: a 25-year-old man presented a tumor in his neck of progressive growth for 12 years. Examination revealed a painless left submandibular tumor, mobile and soft. The ultrasound showed its cystic nature and a computed tomography scan showed a non-enhancing tumor of its wall to intravenous contrast. The cyst was resected with an approximate size of 10 × 6 × 4 cm and a thick yellowish-brown liquid inside. Histopathology of the specimen revealed a cyst lined by stratified squamous epithelium, orthokeratin laminated fibers, and connective tissue without skin appendages.
Discussion: the epidermoid cysts constitute < 0.01% of all cysts in the submandibular region of the neck. The congenital origin, characterized by the absence of previous trauma or surgery to explain their appearance, is even rarer than acquired origin. The surgical resection is the treatment of choice. Cysts located below the mylohyoid and geniohyoid muscle must be removed by an external approach in the neck.
Conclusions: this case is relevant because of the absence of an obvious cause to explain the submandibular implantation of epidermis, so that its origin was probably congenital.
