Fifteen-years experience in the surgical treatment of hyperparathyroidism

ABSTRACT
Objective:The purpose of this work is to present the surgical experience in the treatment of primary hyperparathyroidism.
Setting:Third level health care hospital.
Design:Restrospective, transversal, observational, descriptive and open study.
Patients and methods: This study was performed between February 1989 to march 2004 in the department of surgery at HE CMN la Raza. Age, sex, clinical picture, methodology of study, pre and postoperative calcium and phosphorus serum levels, surgical management, location of the affected gland, and morbi-mortality were analyzed from the clinical records.
Results:Two hundred and ninety cases were recorded in the study period, where 75% were women, and 25% were men. Mean age of presentation was 48.2 years. Clinical picture was characterized by renouretheral lithiasis in 80% of the cases. Serum calcium and phosphorus were elevated in 100% and 89% of the cases, respectively; with a mean value of 11.98 mg/dl, and 2.02 mg/dl, respectively. Pathohistology found adenoma in 80% and hyperplasia in 19.5% of the cases; and the gland most commonly affected was the lower right gland in 42% of the cases.
Conclusion:Primary hyperparathyrodism is an uncommon disease in our community. Time and early identification should avoid secondary complications due to hypercalcemia. Surgical management performed by experienced surgeons should reduce complications to a maximum.
