ABSTRACT

An analysis of the different features that must to be into account in the shape of antimicrobial policy in a complex hospital was conducted. According the experience of our institution, authors express the complex problem related to the way in which an expert group must to advice to different services of our hospital, considering divers factors such as the quality, economy and effectiveness in choice of the different antimicrobials used nowadays. Phenomenon of bacterial resistance is analyzed and the way of functioning of Antibiotics Committee of our institution is showed. The criteria of Evidence-based Medicine versus Experience-based Medicine along all years of scientific praxis are open to criticism in relation to this field into a complex institution where surgical interventions are carried out including a very high percentage the transplantation method. In a brief way, we will refer to antimicrobial prophylaxis in surgical services taking into account the significance of this feature in prevent ion of postoperative sepsis. We conclude showing the experiences achieved during the past 20 years.
