Hospital undernutrition in geriatric patients and concordance between diagnosis instruments

ABSTRACT

Introduction. Morbidity and mortality in the elderly is greatly increased when they are hospitalized and a consequent deterioration in their nutritional status. We compared one of the tools most commonly used in Mexico, the Subjective Global Assessment (SGA), with two other diagnostic methods of malnutrition in a group of elderly people admitted to a hospital. For that reason Subjective Global Valuation (SGV) was compared with other two methods in order to detect undernourishment in a group of elders entered a hospital of second level.
Objective. Compare the frequency of undernourishment using three validated instruments.
Materials and Methods. It was applied the Filter of Nutricional Control (CONUT ®), the VGS and Gold Standard-Villalobos-Gamez et al (GS-Villalobos) to a random sample of 80 patients majors of 60 years admitted to the Internal Medicine Service of the Regional Hospital “Merida ISSSTE” in Merida, Yucatan, Mexico.
Results. VGS diagnosed a 49% of cases of undernourishment, the CONUT ® 60% and the GS-Villalobos 58%. Relevant concordance was observed between GS-Villalobos and CONUT ® (0.81) and VGS (0.72). VGS had a relevant concordance with CONUT ® (0.67).
Conclusions. Frequencies of undernourishment were elevated. SGV, CONUT ® and GSVillalobos were suitable instruments to detect undernourishment in geriatric patients. We propose the use of GS instrument due to its easy applicability.
