Soporte nutricional enteral personalizado en el hogar: Hacia una integración en los cuidados nutricionales ambulatorios

ABSTRACT

Background: Home enteral nutrition (HEN) is an effective way for treating the chronically ill child with an increased nutritional risk. Objective: To show, by means of a methodology integrating the primary and secondary levels of Health Care, that home personalized enteral support is a viable alternative for HEN in Pediatrics. Patients and methods: Between November 1rst, 2009 and November 23th, 2010 (both included) 15 children (Ages: 1 – 18 years) where included within the Personalized Enteral Support Program of the PENU Pediatric Enteral Nutrition Unit of the “Juan Manuel Márquez” Teaching Pediatric Hospital (Marianao, Havana City, Cuba). Children were discharged following the home admission paradigm with a program of activities and cares to conduct at home by their duly trained relatives. The PENU was in charge of supplying the required resources, and verified the continuous fulfillment of the program of nutritional care. Results: Home personalized enteral support by means of integrating the primary and secondary levels of Health Care served to reduce the frequency of chronic malnutrition in 33%, and significantly shorten hospital length of stay, with a better management of available resources. Conclusions: Home personalized enteral support is a viable alternative for enteral nutrition in chronically ill patients with nutritional deficiencies, thus allowing the improvement of nutritional state and quality of life, and a shortened hospital length of stay.
