Estado de los conocimientos sobre Alimentación y Nutrición entre los médicos de un hospital verticalizado en la atención de adultos

ABSTRACT

Undernutrition affects one third of the patients admitted to the Clinical Surgical “Hermanos Ameijeiras” Hospital (Havana City, Cuba). Hospital Malnutrition Study revealed that recognition of undernutrition associated with disease is inadequate, implementation of artificial nutrition schemes is insufficient, in spite of the fact that patients with an indication for them are few in number; and the act of hospital dietetic prescription fails in inform about the patient’s nutrients needs, texture/consistency of foods to be served, and restrictions to be introduced in the diet due to the primary health problem. It has been perceived that previously annotated failures can be explained due to poor knowledge that medical care teams have on minimally required issues of Food and Nutrition. Hence, an ad hoc survey oriented to measure selfperceived degree of knowledge on Food and Nutrition, use of this knowledge in the process of hospital dietetic prescription, and inclusion of a nutritionist within the medical care of the patient was administered to 147 physicians selected from the hospital surgical and clinical services. Fifty-three point seven percent of the surveyed physicians referred to have knowledge on Food and Nutrition. Thirty-four point two percent of them declared they estimated patient’s nutrient requirements as part of the hospital dietetic prescription process. Only 10.0% of the surveyed physicians referred that they consult with a nutritionist about the best nutritional intervention in the patient. At the current moment, the physician’s knowledge is insufficient for dealing with the nutritional and metabolic cost the disease imposes upon the patient’s health status. That might explain, in part, the situation revealed by the Hospital Malnutrition Study. The implementation and conduction of a hospital Continuous Education System on Food and Nutrition offering the acting physician knowledge and tools for dealing successfully with undernutrition associated with disease is urgently required.
