Retrospective study about the efficacy of the Tübingen Torte’s technique for basal cell carcinoma compared to simple resection

ABSTRACT

Basal cell carcinoma is a malignant destructive capacity, ranks in frequency of all cancers in Europe and United States. Although this tumor rarely metastasizes, it is an economic burden and a public health problem. Treatment goals are to make a total tumor resection without sacrificing healthy skin unnecessarily with an objective and not blindly. The choice of treatment depends on many factors giving priority to aggressiveness, location and patient comorbidities. The study compares the results between simple excision technique vs Tübingen Torte a micrographic surgery variant.
Micrographic surgery devised by Frederick Edward Mohs, American surgeon (1910-2002) in 1941 raised the innovation of the resection of tissue in series with histological control. One method is the technique of Tübingen Torte, proposed in 1984 by Helmut Breuninger, focusing on the display of vertical and horizontal surfaces. Few studies evaluating the efficacy and outcomes in terms of patient satisfaction with techniques such as modified micrographs of Tübingen Torte pretending to be simpler and cheaper. The aim is to determine and compare the recurrence of two series of cases of basal cell carcinomas managed with simple resection technique Tübingen Torte. Assess patient satisfaction and recurrence rate.
Materials and methods: Included patients with basal cell carcinoma from January 1993 to January 2011 histologically verified. Statistical analysis was performed using SPSS 18 with X2 test considered statistically significant at p ‹ 0.05.
Results: Of 223 patients with 277 basal cell carcinomas, we found only one recurrence i1.9% in Tübingen technique.
Discussion: This study presents typical biases to be retrospective. The characteristics of our case series are very similar to those reported in the literature. There is satisfaction with the cosmetic result. We recommend the randomized trials, preferable double blind..
