Maxillary sinus lift and bone compression for placement of dental implants.

ABSTRACT

Tooth loss causes bone resorption over time in edentulous areas due to the lack of bone stimulation. Dental implants are the treatment of choice to replace the function of missing teeth. However, there are anatomical limitations caused by tooth loss, such as maxillary sinus pneumatization and the lack of elevation between the alveolar bone and maxillary sinus, which prevent implant placement. Nevertheless, the jawbone possesses structural features that allow bone condensation and provide primary stability of the implant. In this clinical case report we describe a procedure to retain bone by compressing it laterally and axially to create an ideal implant bed. Following initial osteotomy, bone condensation was performed using osteotomes of increasing diameters with a ratchet for the maxillary sinus lift and to allow the placement the dental implants. When performed correctly, this technique can create a sufficient quantity and quality of bone for the placement of implants, thus allowing us to increase the volume level of the alveolar bone crest around its base and to restore an edentulous space prosthetically, so avoiding the need to prepare healthy adjacent teeth.
