Deceased-donor kidney allocation by a score system for patients in a transplant waiting list

ABSTRACT

Choosing the recipient of a deceased-donor kidney transplant is a complex process involving multiple factors. Finding the balance between medical efficiency and social equity in organ allocation is no simple task, therefore several variables must be taken into account. With that objective, a score-system was designed that allows kidney allocation to the most adequate recipient in each case. Among factors considered were: waiting time in list, reactive panel of antibodies percentage, dialysis time and donor-recipient age. A computer program was created that allows periodic update of each patients’ transplant protocol information, and automatically calculates, according to each individual score, the order in which donated kidneys must be allocated, permitting organ traceability by government authorities supported by the score obtained in each case. Allocating organs by an objective and systematized method to the «fittest» recipient can lead to a more efficient and fair process, obtaining always the best possible survival rates from each kidney, regarding principles of efficiency and equity.
