Therapeutic management of the keratocyst: Review and clinical case presentation

ABSTRACT
The keratocyst or keratocystic odontogenic tumour (KCOT) is morphologically similar to other odontogenic cysts but is characterized by a more aggressive and recurrent behaviour which has led to discrepances when choosing a suitable therapeutic option. There are several treatment options for the management of these lesions: marsupialization, enucleation, adjuvant measures (such as Carnoy solution, liquid nitrogen or osteotomy) and bone resection. Actually, current trends prefer conservative treatment when lesion size and characteristics allows this, achieving satisfactory results, low level of recurrences and a low morbidity with these techniques. In this paper we present the case of a lesion which occupied the entire left hemimandibular portion and was treated by segmentary mandibulectomy from right parasymphysis to left subcondylar region and immediate reconstruction with free fibula flap.
