Laryngeal scleroma: case presentation and review of the literature

ABSTRACT

Rhinoescleroma is a chronic granulomatous infection of the upper respiratory tract caused by Klebsiella rhinoscleromatis. It predominantly affects women (5:3) with an incidence between 15 and 35 years. Ninety five percent of cases are localized to the nasal cavity and 15 to 80% percent involve the larynx, with the subglotis being the most common affected site. It is divided clinically in three stages: atrophic phase, with crust formation, the proliferative or granulomatous stage with obstructive nodules and the sclerotic or fibrotic stage with laryngeal and nasal stenosis and external deformities. The definitive diagnosis is made by histopathological analysis. Imaging studies are only performed to establish the extension of disease and to evaluate the involvement of the lower respiratory tract. Treatment is based on long term antibiotics, and surgical treatment is only performed in cases of obstructive lesions or cosmetical deformities. We described the case of a patient with rhinoescleroma with laryngeal involvement and a revision of the most updated cases and reports in literature.
