Distal Nail Embedding. Report of 24 cases

ABSTRACT

Background: The distal nail embedding is a common complication of nail avulsion; as the nail plate is blocked by the hyponychium that gives rise to the formation of an edge or ring that interferes with the newly formed growing nail.
Objective: To identify the causes of distal nail embedding.
Methods: A descriptive study of 24 cases observed in private practice and patients at the Hospital General de Enfermedades IGSS. They were seen in September 2010, in order to study the presence of distal nail embedding. All patients underwent dermatological examination of toenails, with emphasis on their distal portion, overlooking any other nail diseases.
Results: We studied 24 patients, 15 (62.5%) male, and 9 (37.5%) female. The affected toes were: first toes bilaterally 11 (45.83%); first left toe 6 (25%); first right toe 5(20.83%); first and third right toes 1 (4.1%), and first and second left toes 1 (4.1%). The causes associated with distal nail embedding were: nail removal in 9 cases (37.5%); onychomycosis 7 (29.16%); nail pterygium in 3 (12.5); onychocryptosis 2 (8.33%); onychodystrophy 2 (8.33%), and onychogryphosis 1 (4.1%).
Conclusion: Avulsion of the nail plate causes distal nail embedding, and at the same time onychocryptosis. It is important to avoid total nail removal to prevent this complication.
