Congenital adrenal hyperplasia in a patient undergoing cesarean section

ABSTRACT

Introduction: congenital adrenal hyperplasia belongs in a group of congenital autosomal recessive diseases in which an error occurs in the adrenal steroid genesis due to deficient activity in one of the enzymes required for the synthesis of cortisol, with a compensating increase in pituitary corticotropin and hyperplasia of the adrenal cortex.
Objective: present the anesthetic management of a patient with congenital adrenal hypertrophy in the course of a cesarean section.
Clinical case: a case is presented of a 22-year-old patient with 38.2 weeks pregnancy who had been diagnosed with congenital adrenal hyperplasia 4 years before. Successful cesarean section was performed with epidural anesthesia.
Conclusions: the disorder constitutes a high anesthetic risk. Very little information is available in the bibliography about the anesthetic procedure. Each patient should be evaluated individually by a multidisciplinary team. Collective decisions will lead to satisfactory results.
