Defect in occipital region due to myiasis. Trapezius flap reconstruction. Case report

ABSTRACT

It is not common that myiasis infestation is present in the posterior region of the skull, being the lower extremities with prevalence in endemic areas, the main locations in frequency. The diagnosis is made by direct visualization of the larva. The multidisciplinary approach is essential for the great defect and complications that may arise. We present a 71 year-old patient with a history of poorly controlled diabetes mellitus and social neglect, who arrived at the emergency room with ulcerative lesion with seven years of evolution in occipital region of about 4 cm, with progressive and painful growth. He underwent surgery in which larvae infestation was found; Surgical cleaning, debridement and antibiotics were used with clinical improvement. In a second surgery, the defect was reconstructed with a trapezoidal flap with skin island, showing an adequate response to treatment without complications. It was possible to cover the defect in the occipital region using the trapezius muscle flap of the proper length, richly vascularized and minimal donor site morbidity.
