Clinical radiological correlation with anatomic-pathologic results of lymphadenectomy in patients with oncologic tongue lesions

ABSTRACT

Objective: The aim of this study was to correlate the anatomopathological findings of lymphadenectomies in patients diagnosed with tongue carcinoma with the preoperative clinical and radiological data obtained through cervical palpation and computed tomography (CT), assessing the predictive capacity of those studies in the detection of lymph node metastasis. Material and methods: A retrospective study of 113 patients diagnosed with lingual neoplasia during the period from 2008 to 2013 by the Oral and Maxillofacial Surgery Department of the Hospital General Universitario Gregorio Marañón. Of these, 66 patients met the inclusion criteria; the rest were excluded. Results: Considering the performed lymph node dissections, histological findings demonstrated tumoral infiltration (N+) in 31.82%. The sensitivity of both the cervical palpation and CT was 76.20%, while the specificity of the cervical palpation and the CT were 77.78% and 80% respectively. Palpation and CT agreed in 72.26% of cases. Both examinations were positive in 35.30% and nodal involvement was demonstrated in 77.78% of these cases through histological findings. Conclusions: The low sensitivity and specificity found in the tests currently protocolized in the detection of metastatic lymph nodes, as well as the high percentage of subclinical neck lymph node metastasis, common in tongue neoplasms, could explain de discordance found in the clinical and histological staging. Consequently, more sensitive and specific tests are needed to enhance the detection of lymphadenopathies. The absence of these exams could explain the prophylactic lymphadenectomies performed.
