ABSTRACT

Introduction. Secondary incisional hernias surgical wounds Mc Burney type, as well as interstitial hernias are both infrequent, developing an interstitial hernia in this type of injury is finding a few times described in the literature. Case report. We present a patient with history of appendectomy, using an approach Mc Burney incision type, evolving with wound infection, wound dehiscence to drain collections is made and found good aponeurosis of the external oblique muscle, clinically there’s no evidence of incisional herniation. A radiographyc finding is discovered as an interstitial hernia at the surgical wound. Abdominal wall plasty was performed without mesh. Wound infection was managed by surgical scrubs and later with use of VAC system. Finally the wound heals by secondary intention. A 12 month follow-up no recurrence data interstitial hernia. Conclusions. The interstitial hernias are uncommon and usually enrolled in a silent way, they are usually diagnosed when complications occur. Its management is controversial if presented in the context of a septic process, with mesh treatment even controversial.
