External otitis caused by fungi. An open study in 36 cases in a general hospital

ABSTRACT

Background: The diagnosis of external otitis is based on the story of the patients with their herotoscopic tests under microscopic control and imaging studies. Aspergillus spp and Candida spp are the most frequent etiologic agents in otomycosis. Isolation of the etiological agent is particularly important for a correct diagnosis.
Objective: To identify external otitis caused by fungi, as well as to detect the most common predisposing factors for otomycosis in patients at General Hospital Dr. Manuel Gea González, in Mexico City.
Material and method: A retrospective, cross-sectional and observational study was done in 36 patients with otitis with fungal structures in direct examination or culture. The presumptive diagnosis of otomycosis was carried out in the service of Otolaryngology and confirmed at the Mycology laboratory of General Hospital Dr. Manuel Gea González.
Results: In 36 cases, the positive direct exam or culture was unilateral, without predominance of right or left ear. 19 women were affected and 17 men; the age varied between 9 at 84, with an average of 39 years. Nine of 36 patients were positive by microscopic direct examination, 6 of 36 for culture and 21 of 36 by both methods. We found a relationship with the history of otitis media in 21 of 36 patients, with a slight predominance of males (12 men and 9 women). Isolated molds were distributed as follows: Aspergillus (n=9), Aspergillus flavus spp (n=5), Aspergillus niger (n=5), Aspergillus nidulans (n=1), Aspergillus terreus (n=1), Absidia spp (n=1); yeasts were: Candida albicans (n=2), Candida spp (n=3).
Conclusions: Aspergillus spp was the most common causal agent of otomycosis, followed by Candida spp. We found history of otitis media in 21 of 36 patients, with an average age of 39 years and both sexes were affected equally.
