Correlation of ovarian morphology and resistance to insulin in patients with polycystic ovary syndrome

ABSTRACT

Background: Polycystic ovary syndrome is a common endocrinemetabolic disorder. Resistance to insulin is crucial in its origin with direct effect on ovarian morphology.
Objective: To determine the relation between ovarian morphology and resistance to insulin in patients with polycystic ovary syndrome.
Material and method: A retrospective, cross-sectional, descriptive study of 97 patients from Gyneco-endocrinology Clinic with polycystic ovary syndrome (Rotterdam criteria). Four groups were formed: A: normal ovarian morphology; group B: volume › 10 cc; group C: › 12 follicles of 2-9 mm of diameter and group D: volume and increased number of follicles. χ2 test was used for statistic analysis.
Results: Mean age of patients was of 26 years (range: 18-43), frequency of resistance to insulin was of 62% (n=61). Resistance to insuline was higher in groups with increased ovarian volume: 19 patients (76%) in group D and 24 patients (64%) in group B (p=0.014). In groups B and D cholesterol LDL ›100 mg/dL was related, as well as index waist-hip (p=0.015 and 0.011, respectively) to resistance to insulin.
Conclusions: Increased ovarian volume, not follicular distribution, has higher relation to resistance to insulin, which is agree with world literature. Prospective studies are required for determining if ovarian volume may be a cardiometabolic risk marker in patients with polycystic ovary syndrome.
