Postoperative complications associated with impacted lower third molar surgery

ABSTRACT
Background: Impacted lower third molar surgery is a routine procedure in the practice of oral surgery. However, there are various complications associated with the extraction of impacted mandibular third molars, the most common being pain, swelling, trismus, hemorrhaging, ecchymosis, dry socket, infection, paresthesia, and difficulty with chewing.
Objectives: The aim of this study was to evaluate the incidence of postoperative complications following impacted lower third molar surgery in patients at the Academic Unit of Dentistry of the Autonomous University of Nayarit, so as to ensure these are taken into account and that the respective precautions are taken, either by attempting to avoid them in the future or to recognize and treat them. Pain, inflammation and trismus are not analyzed in this paper.
Material and methods: A longitudinal prospective study of 38 patients between the ages of 16 and 38 years old (with an average age 23.1) was carried out over seven consecutive days, with follow up examination performed at 15 and 30 days. The postoperative complications associated with lower third molar surgery were assessed.
Results: 38 patients took part; 29 females (76.3%) and 9 males (23.7%) between the ages of 16 and 38 years, with a mean age of 23.16 ± 5.2 years old. The most common complication was difficulty with chewing followed by ecchymosis. Other complications encountered included ulcers or blisters on the lip or oral mucosa, bleeding, exophytic clots, dry socket, infection, and paresthesia.
Conclusions: There is a link between ecchymosis and factors such as sex, tooth type, tooth position (based on Winter’s classification), class and depth of the tooth (according to the Pell and Gregory classification), and tooth section, and a link between age and difficulty with chewing.
