Laparoscopic cholecystectomy with three ports. A modification to the approach

ABSTRACT
Objective: To report the results of using only three trocars, modifying the size and the site the ports in the patient.
Design: It is a longitudinal, retrospective, no comparative, without exclusion criteria.
Setting: 2
nd. Level medical attention hospitals in Cuernavaca, Morelos.
Materials: 318 cholecystectomies were performed by laparoscopy, at the beginning only with 3 work channels: one of 10/12 mm. And two more of 5 mm. Each one placed at the umbilical scar, epigastrium and left flank.
A preoperative study was performed to all the patients that showed some differences in age and sex.
Results: 92.45% of success using 3 trocars diminishing the conversion index to less than 2. The average age was 42.8 years old, operating time of 44.8 minutes, complications were found only 11 patients with, only one death case (0.3%).
Conclusion: The approach with 3 ports is highly effective and safe, letting the surgeon plenty of comfort, the same visual camp compared with 4 ports, maneuvers can be done as well as complementary procedures during the laparoscopic Cholecystectomy, as cholangiographies, biopsies, etc.
