Vesicular lithiasis and situs inversus totalis resolved by means of laparoscopic surgery with needlescopic assistance

ABSTRACT

Objective: To report a case of a 75 year-old-female with chronic lithiasis cholecystitis and situs inversus totalis (SIT) solved by laparoscopic surgery with Needlescopic assistance. Design: Regional Zone Hospital No. 1, “Lic Luis Garcia Tellez”, of the Social Security Mexican Institute at Cuernavaca, Morelos, Mexico. Case description: Is about a 75 year-old-female who presented and abdominal pain, at the hipocondrious of many months of evolution accompanied with nausea, vomiting, jaundice and fever. It was demand the following labs (CBC, UA, LFT, BCH, SE, PT, PTT, sGOT, sGPT and alkaline phosphatase), EKG, X rays and ultrasound were taken establishing the obstructive jaundice diagnosis. It was request an ERCP (endoscopic retrograde cholangiopancreatography), presented in session to listen new opinions, confirming the association of chronic lithiasis cholecystitis and situs inversus totalis, then, the patient was schedule for cholecystectomy and transcistic cholangiography by laparoscopic route with needlescopic assistance. Completing a successful procedure without complications and discharging the patient 24 hrs after the procedure. Conclusion: Is the case of a 75 years-old- female with situs inversus totalis and abdominal pains associated with vesicular lithiasis that ended in laparoscopic surgery prior to complete medical analysis, performing the laparoscopic procedure with needlescopic assistance having a high security range.
