Diagnostic and therapeutic thoracoscopy. Report of three cases

ABSTRACT

Nowadays there is controversy about the ideal surgical procedure for the management of pericardial effusion disease. Advantages and disadvantages depending in the kind of patient. The thoracoscopy have excellent results in terms of feasibility, less morbi-mortality and the easily reproducible technique in contrast with the conventional procedures. We describe three cases of tuberculosis pericardial diseases in which thoracoscopy showed clear advantages to perform pericardial window, exploratory thoracoscopy, diagnostic and therapeutic procedures. We observed an the evident change of the clinical course of the patients without related procedure complications, less postoperative pain and no recurrence at the time of report. Thoracoscopy have been a useful tool as diagnostic procedure, therefore we conclude it is an excellent therapeutic option on patients with severe pericardial effusion disease that their clinical performance is good enough to tolerate a general anaesthesia and unilateral lung ventilation, with a life expectancy of over four months or possible concomitant pleural-lung disease. Also in cases with posterior loculated effusions that have shown difficult approach with the conventional procedures.
