Psychiatric comorbidity and perception of body self-image for daydreams, in a sample of transgender people

ABSTRACT

Introduction: In the present study it was explored the validity of dreams awareness in the situation of first person in transsexuals, in order to develop some diagnostic tools for to diagnosis of such conditions.
Methods: A total of 43 persons were studied: 36 male to female (MtF) and 7 female to male (FtM) transsexuals. The full procedure was explained and the informed consent was obtained before enter into the studio. Transsexual persons were assessed with the DSM-IV-TR criteria for gender dysphoria disorder. Also, they were diagnosed with the structured clinical interview questionnaire (SCIDS panish version) to assess comorbid psychiatric disorders. Lastly a sleep and dreams log were instructed to be filled out every day. The dreams log emphasizes in their report in a narrative form, addressing in what kind of gender they appear more often in their dreams. TS person in this study did not receive yet hormonal treatment for their transitions.
Results: Thirty-three TS people had some type of psychiatric diagnosis. In rank of order of prevalence, were: major depression, anxiety disorders, alcoholism, social anxiety, anorexia nervosa and borderline personality. Sleep disorders were chronic insomnia and sleep fragmentation. TS subjects were divided in two groups. Those who noticed their condition at early age (‹15 years old); and those who were aware of that later in life (› 15 years old). The first group had 23 people female transsexuals (MtoF), reported more dreams in which they perceived themselves coherent as the gender that they feel to belong to. Also they have less sleep efficiency.
Conclusions: The use of the analysis of dreams from a perspective of first person narrative may be useful to evaluate the diagnosis of transsexualism in people who may not be sure of their condition.
