ABSTRACT

Postoperative bile leak following laparoscopic cholecystectomy is one of the described complications of the approach, although infrequent, it is seen more often than in open surgery.1-5,15 Generally it is due to an unrecognized lesion of the main biliary tree.1,2 However, the presence of accessory ducts, with or without apparent lesion, poor clipping techniques, tear or perforation of the cystic duct have also been described.11-14 Postoperative bile leak can usually be diagnosed and treated conservatively with expectant management, especially in those cases where routine drainage of the gallbladder bed is left in place. This usually allows prompt recognition, and provides adequate treatment, due to the fact that most leaks resolve spontaneously. Endoscopic procedures have gained popularity due to their relatively low morbidity and effectiveness. We present the case of a female patient who underwent laparoscopic cholecystectomy and presented with a biloma in the late postoperative period, secondary to a bile leak from a previously identified, clipped accessory duct. Endoscopic and surgical management are described.
