Diffuse alveolar hemorrhage: frequency, cause and outcome

ABSTRACT

Background: Diffuse alveolar hemorrhage (DAH) is a clinical pathological syndrome that describes the presence of erythrocytes in the alveolar space; these are originated from pulmonary capillarity.
Objetive: To describe the frequency, etiology and outcome of DAH in the ISSEMyM Hospital Center, Toluca, Mexico.
Material and Method: A descriptive and retrospective study was done based on the review of medical records from March 2008 to March 2013 to classify the DAH cases according to current criterion.
Results: Fifteen cases of DAH were identified. The main causes were autoimmune diseases: 40% lupus erythematosus (LES) and 40% ANCA’s associated vasculitis (AAV); 46% of patients were admitted to intensive care unit, and they required invasive mechanical ventilation (IMV), with an average stay of ten days and mortality of 20%.
Conclusion: The main causes of DAH were autoimmune diseases (in the same proportion LES-AAV), showing a mortality of 20%. The only association with death was the IMV necessity.
