ABSTRACT

Achalasia is a chronic and progressive esophageal motor disorder manometrically characterized by loss of peristalsis of the distal esophagus, failure of the lower esophageal sphincter to completely relax with deglutition, and elevated baseline intraluminal esophageal pressure. This disease should be suspected in patients with dysphagia to solids and liquids and in those with regurgitation unresponsive to a trial of proton pump inhibitor (PPI) therapy for four weeks. Manometry is required to establish the diagnosis of achalasia. Without treatment, patients with achalasia develop progressive dilation of the esophagus.
