Effusive-constrictive pericarditis due to Mycobacterium tuberculosis. A case report and literature review

ABSTRACT
The effusive-constrictive pericarditis is a rare presentation of tuberculous pericarditis associated with pulmonary tuberculosis (2%); the case presented here has confirmation with adenosine deaminase, polymerase chain reaction of tissue and pericardial biopsy. A 34 year old woman with symptoms with two months of evolution characterized by sudden deterioration in functional class, thermal hikes, night sweats and consuntional syndrome. Initial echocardiogram with 700 mL of pericardial effusion, pericardial thickening of 2.62 cm, paradoxical septal motion, left ventricle ejection fraction 51%, systolic pulmonary artery pressure 27 mmHg. Underwent pericardial window, managed with steroid (11 weeks of prednisone) and antibiotics for six months, nevertheless complicated with constrictive pericarditis that required pericardiotomy, with adequate subsequent evolution, for a year of follow-up persisted asymptomatic.
