Laparoscopically assisted vaginal hysterectomy. Experience of 139 cases (1994-2001)

ABSTRACT
The laparoscopy had dramatically change the general surgery, and the hysterectomy was not the exemption. After Reich’s report in 1989 thousands of articles had flooded the world literature supporting and criticizing the procedure. Here we present a retrospective analysis of our experience with 139 cases from 1994 to 2001. Our results showed that the main indication for the procedure were the fibroids, responsible for the 79% of the cases. There was not a difference in regard to the preoperative hematocrit and the postoperative (mean 12.4 vs 11.2 respectively). The mean hospital stay was 2.43 days per patient and the average uterine weight was 213 g. We had a complication rate of 12.29% with two major complications, one patient developed a pulmonary embolism and another one had an inadverted sigmoid injury which required a colostomy and was reanastomosed afterwards with a good outcome. Only one patient required to be converted to a laparotomy due to a large uterus and technical difficulties. We concluded that the laparoscopically assisted vaginal hysterectomy is a good substitute for the abdominal hysterectomy keeping in mind the potential major complications that could develop with the technique and the way to avoid them.
