Handling of the pericardial effusion by endoscopic surgery

ABSTRACT

Objective: To report the initial experience in the evacuation of the pericardial effusion by the endoscopic approach in hospitals from the second level of attention. Setting: Regional Zone Hospital No.1 “Luis García Tellez” from the IMSS, General Hospital of Cuernvaca, “Dr. José G. Parres”, From the Health Services in Morelos and Privates Hospitals from Cuernavaca, Morelos. Design: Retrospective, non comparative. Material and methods: A total of 9 pericardial effusions in the same number of patients were evacuated by the video assisted thoracic surgery (VATS) approach, from april of 2000 to october 2002. The analyzed factors were: age, gender, hospitalary stance preoperative and postoperative, morbimortality and others. In all the cases a preoperative protocol were established. Results: Nine patients undergone surgery, seven (77.77%) female and two (22.22%) male, with an average age of 51 years old, in six patients the preoperative diagnosis of cardiac tamponade was established, we had one death in this cases and there are no complications in the rest of the eight cases after their hospitalary deliver. Conclusions: We considered that the thoracoscopic approach offers enormous advantages over the conventional surgical methods for the treatment of the pericardial effusions in patients hemodynamically stables to sove this problem with a better therapeutic option.
