New technique for placing cartilage graft in the nasal columella in orthognathic surgery

ABSTRACT

A new surgical technique is described as ideal for an additional nasal aesthetic refinement option in orthognathic surgery using septal cartilage graft in the horizontal maxillary osteotomy cranial intrusion of 5 mm, cutting the excess base cartilaginous septum, obtaining a graft that is placed on the anterior edge of the septal cartilage through the dissection of the mucosa toward the columella to the nasal tip, separating the fibrous bands that articulate the caudal septum and the medial crura (this region is named «membranous septum»); this creates a pocket in the nasal columella, where the previously developed graft is placed; the mucosa is sutured to ensure settlement in the anterior nasal spine of the maxilla. The technique of placing cartilage graft in the anterior septum in orthognathic surgery is a proprietary design implemented since 2012 and has been used in twelve patients with excellent results.
