Dental needle broken in the pterigomandibular space. Migration to the carotid space

ABSTRACT

We present a case in which a needle, broken in the left pterygomandibular space, migrated to the carotid space over a few weeks. We also mention the various symptoms that manifested during its trajectory. The initial panoramic radiograph showed the needle in the left pterygomandibular space, with its tip very close to the anterior border of the mandibular ramus; a few days later, a second panoramic radiograph demonstrated the needle’s tip far from its initial position, but still in the pterygomandibular space. A few weeks later, the patient’s symptoms changed and a computerized tomography demonstrated the needle to be in the left carotid space. During surgery, in which we used an operating microscope, we retrieved the needle, which was found to be in close relation to the internal carotid artery, almost in contact with the cranial base.
