Levetiracetam in neonatal seizures: Two years of experience in tha National Institute of Perinatology

ABSTRACT

Introduction: Diagnosis and treatment of neonatal seizures is a challenge for physicians. The gold standard for diagnosis is videoelectroencephalogram. We know the low efficency and the possible hazardous effects of the antiepileptic drugs in inmature brains. Recently we have use levetiracetam in neonates.
Objective: To report our experience with levetiracetam in neonatal seizures.
Methods: This is an observational, retrospective, cross-sectional study. We analize the files of twelve neonates treated with levetiracetam.
Results: The treatment used in our patients was according to the Perinatology National Institute Manual of Procedures in Neonatology. Phenytoin was the first line treatment, followed by midazolam, lidocaine, phenobarbital and levetiracetam in second and third line in all cases. The intravenous loading dose was 20 mg/kg, given in slow infusion during 60 minutes. We had satisfactory response with suppression of seizures in 75% of cases.
Conclusions: Levetiracetam is being used recently in neonates, offering a novel mechanism of action, with certain pharmacokinetics advantages. This observational study provides a proof of concept for this age group. We have intended to continue with and experimental trial of this antiepileptic in neonates.
