ABSTRACT

Arrowsmith described the first case of actinomycosis; however, the beginning of the antibiotic era ended infections due to actinomicets. The most frequently isolated organism is Actinomyces israelii. Clinical manifestations are variable and can mimic a number of diseases. Primary actinomycosis can occur on immunosuppressed patients, and secondary disease occurs because of trauma and dental manipulation. This paper reports the case of a 27-year-old male, who presented with a history of sore throat and changes in the voice for 2 months. On admission, he presented shortness of breath and limited oral opening, as well as increased volume of right palate. CT scan showed a mass on the right parapharyngeal space, displacement of the soft palate and irregular right tonsil. He was taken to the operating room finding loss of architecture of the tonsil, necrotic material and fetid gas. The surgery was uneventful and patient was discharged. This clinical presentation can be mistaken with tumors of abscess on low prevalence groups; the low probability of detecting the diagnosis, because it is unusual, can delay the treatment.
