Laparoscopic pyloromyotomy, a good option?

ABSTRACT

Background: The surgical approach for infantile hypertrophic pyloric stenosis (IHPS) has evolved; currently laparoscopic approach (LA) is proposed as a cost-effective option. The goal is to show our series of cases including 2 cases operated with 5 mm equipment, and review of the current literature. Material and methods: A review of the first 7 cases operated by LA at the National Institute of Pediatrics (INP) diagnosed with IHPS was performed. The first 2 cases were operated with 5 mm instrumental and the other 5 with 3 mm. Results: In 7 patients the procedure was successful without complications during of after surgery. The average operative time was 41.4 minutes (range 20-50 min), the resumption of orally was 2 to 17 hours, the average discharge time after surgery was 1.7 days. Cases operated with 5 mm equipment showed no postoperative vomiting. Conclusions: LA results are strongly influenced by an undeniable learning curve of each surgeon, each hospital and the instruments used; however, the advantages demonstrated and the results of recent cost-effectiveness studies, tilt the balance toward the LA.
