Open fractures: incidence rate of infection. Report of 273 cases

ABSTRACT

Introduction: The complication rate reported worldwide for open fractures is 20%; infection is the most common complication, with a global incidence rate of 3-4% and up to 8% in underdeveloped countries. The incidence rate of infection in open fractures is directly related to their correct treatment and to the type of exposure. Objective: To identify the incidence rate of infection in open fractures and identify the incidence rate of infection according to the type of exposure. Methods: A retrospective, longitudinal, observational and analytic study was developed, including a total number of 273 patients. The type of exposure was identified based on the Gustilo & Anderson classification of open fractures, and their course was assesed identifying cases of infection, including surgical wound dehiscence, wound infection, osteitis and septic pseudoarthrosis. The infection rate according to the type of exposure of the fracture was measured with univariate analysis, and the association of the variables of interest was established using a bivariate analysis with the χ2 statistical test. Results: We identified a global infection rate of open fractures of 8%. According to the type of exposure, the incidence rate of infection ranged increasingly from 0% (type I) to 17.39% (type IIIC). The type of exposure of open fractures was statistically significant (p = 0.04) for the development of infection. Conclusion: According to our date study the incidence rate of infection in open fractures is high according to worldwide literature, and is directly related to the type of exposure.
