La referencia a trasplante renal en el IMSS: un área de oportunidad para la mejora

ABSTRACT

Introduction: Renal function therapies provide life support, but offer a high possibility of complications, and deterioration of quality and reduction of life expectancy. Renal transplantation grants the greatest rehabilitation, the possibility of a normal life, and family, social and labor reintegration. Objective: To identify the main characteristics of potential kidney recipients in the Mexican Social Security Institute over a period of two years. Material and methods: Retrospective and transverse two-year study, including references to renal transplantation of second level hospitals during 2014 and 2015. It did not include subjects older than 65 years of age. They were identified by delegation, gender, age group, cause of chronic renal failure (CRF), and status of dialysis therapy modality. Results: 5,484 patients were referred: 2,479 in 2014 and 3,005 in 2015; 3,369 were males (55%) and 2,115 females (45%). The frequency by age group was from four to 16 years (2%), from 17 to 27 (39%), from 28 to 38 (22%), from 39 to 49 (20%), from 50 to 59 (13%), and 60 years or older (4%). The reference showed an annual increase of 9.75% overall, 304 male cases (9%) and 222 female (10.5%). Jalisco was the delegation with greater reference in the two years, followed by Coahuila, Puebla and the Distrito Federal Norte; the majority of delegations showed an increase in the second year. Regarding the dialysis modality, the percentage of patients who underwent peritoneal dialysis was 56%, hemodialysis 37%, and pre-dialysis (7%). Throughout the period of study, the reference of patients of all modalities increased. Conclusions: The reference to renal transplantation shows a continuous increase; pre-dialysis patient become important and are referred more frequently. To benefit them, transplant surgery must be supported.
