Simultaneous acute rheumatic fever and septic arthritis by Streptococcus pyogenes in a preschool child

ABSTRACT

The WHO has estimated that approximately 500,000 people suffer annually from an acute attack of rheumatic fever (RF); two thirds of them will develop rheumatic heart disease. S. pyogenes is the causal bacteria of this disease. RF is secondary to an acute streptococcal infection; it is cataloged as an autoimmune disease characterized by kidney, heart and skin inflammation. The age of presentation associated with increased incidence is between five and 15 years; only about 5% of the cases occur in children under that age. We present a case of a 35-month-old patient with simultaneous acute RF and septic arthritis by S. pyogenes who developed myocarditis, tricuspid insufficiency and pericardial effusion.
