ABSTRACT

Introduction: Donors of whole blood and blood components are vulnerable to declining iron stores. Goals: To evaluate, through the serum ferritin quantification, the iron reserves and their relation to the estimated losses of serum iron and erythrocyte mass in different types of donors. Material and methods: A descriptive, observational study was performed on 98 men donating clustered blood in three groups: first-time and habitual whole blood donors, and apheresis plasma donors. Their serum ferritin concentration was quantified and losses of erythrocyte mass and serum iron were estimated. Ferritin values were compared for each group and related to estimated losses. Results: Whole blood first-time donors showed higher mean ferritin values than the rest of the individuals. One-tenth of the total plasma and repetitive blood donors had ferritin levels below the values considered normal. Repetitive blood donors can lose up to twice erythrocytes and iron compared with plasma donors. Conclusions: Impairments in iron stores are similar in habitual and plasma donors, despite differences in the estimated amount of mineral loss. Recommendation: To protocol additional tests in the selection of habitual blood and components donors to monitor subclinical states of iron deficiency.
