La ventilación no invasiva en el destete de los pacientes con enfermedad pulmonar obstructiva crónica

ABSTRACT

Introduction: the release of mechanical ventilation is a challenge for Intensive Care and Emergency Specialists; it is the chronic obstructive pulmonary disease one of the most important in this sense. The role of noninvasive ventilation in the post-intubation period is important in the prevention of complications and the recurrence of respiratory failure. Objective: to verify the usefulness of noninvasive ventilation after extubation in patients with chronic obstructive pulmonary disease exacerbated in the Intensive Care Unit of the “Arnaldo Milián Castro” Hospital. Method: a prospective, observational, longitudinal, study was performed between December 2013 and July 2016. The study population consisted of 20 patients requiring mechanical ventilation, who were extubated noninvasive ventilation. The results obtained were compared with those of a historical control group of 20 patients who were retrospectively and consecutively performed from November 2013 to January 2011, to whom the protocol was not applied. Results: noninvasive ventilation applied after extubation to patients with exacerbated chronic obstructive pulmonary disease decreases the incidence of pneumonia associated with mechanical ventilation and reintubation, and thus total ventilation time, as well as the need for tracheotomy. Conclusions: the application of this ventilation regimen in patients with chronic obstructive pulmonary disease decreases mortality.
