ABSTRACT
We report the case of a female geriatric patient, with the presence of pain and with an important increase in the shoulders, which was referred to the rehabilitation service under the diagnosis of massive bilateral rotator cuff lesion. Clinical assessment, as well as arthrocentesis, laboratory and cabinet studies, allowed the diagnosis of a bilateral subacromial-subdeltoid massive bursitis, with the presence of the sign in image, called «rice granules». The review of the literature was carried out at the international level, being detected that only two similar cases have been reported, while in Mexico, it would be the first report. The treatment by physical therapy and aspiration of the synovial fluid of the subacromial-subdeltoid bursa was not satisfactory, and it must be turned to the assessment of surgical management, which is the treatment of choice.
